Register for Lead Service Line Replacement

The City is currently accepting registrations for Phase I of the program. If you have received a letter from the City offering to replace your lead service line, please register by one of the options below.

If you answered “Defer” or “No” above, would you like to participate in the City’s lead sampling program?
Participation requires you to collect up to two (2) water samples each year with direction from the Department of Water and Sewer. Each sample result will be provided to you by the City after each sample test is completed. Up to 150 new volunteers per year will be randomly selected to participate in the lead sampling program. By participating in the lead sampling program for at least one (1) year, you may be eligible for a further reduction in the lead service line replacement cost to be performed in a later phase.

Download the Registration Form. You can digitally fill and sign the form using the Adobe Reader application. You can also print, fill, and scan or take a photo of the form.
